OASIS Code List Representation TC

This version, like the previous version, only makes changes to section 4. 
The documentation (taken from the Schema) has been improved, although not
all of the documentation has yet been reviewed (but the majority has been).
 Note that section 4 is a reference section, and will make more sense once
the earlier sections are completed to provide a more holistic view of
genericode and how it is used.
