1.1 XSD and 1.0 DTD update

    Hi everyone,

The "extype" attribute was missing from the 1.0 DTD and the 1.1 XSD files,
both documents have been now updated. Thanks to Ramesh Kumaraswami and his
colleagues for finding the problem.
