OASIS eXtensible Access Control Markup Language (XACML) TC

This is a new version of the core 3.0 draft.

The main change is that I have migrated the whole text to the current OASIS
template. Because of this there is no diff document this time.

I have not changed the technical content, but I have corrected those typos
and errors I found while working. For instance there were many references
to XACML 2.0, and other obvious errors. I also fixed the couple of issues
Rich found recently.

I will also send a separate email with a couple of issues I found.
