Submitter's message hi, due to the unexpected issues with the HTML filter, i've tried to get the XHTML filter to work, which at least can create hyperlinks for all the cross references.
please check if there's any obvious issues with it, and if it's better or worse overall than the previous HTML file.

XHTML filter needs a tweak because there's too many headings in the file.
