I followed your advice and did the commenting (and some more) and to my happiness suddenly it worked.
I actually suspect that the multiple ids did have something to do with my problem all along but it did not reveal itself.

I have already filed a bug report for this (http://sourceforge.net/tracker/index.php?func=detail&aid=1859831&group_id=21935&atid=373747).
To work around it, in each of these two templates I commented out the second <xsl:call-template name="anchor">.
