    Using the 1.74 html stylesheets, my docbook preface info given below results
    in:

    John Q Foo Corporation Smith

    CTO
    Foo Corporation




    <preface>
    <beginpage/>
    <prefaceinfo>
    <author>
    <firstname>John</firstname>
    <surname>Public</surname>
    <othername role="mi">Q</othername>
    <affiliation>
    <jobtitle>CTO</jobtitle>
    <orgname>Foo Corporation</orgname>

    </prefaceinfo>

    ...
    </preface>

    Got the same result with the latest snapshot.
