I have tried to address issues raised in email discussion.

The document named DITA 1.1 Issue #45a: Add page range indexing elements
(IssueNumber45b.html) has been submitted by Mr Christopher Wong to the
OASIS Darwin Information Typing Architecture (DITA) TC document
repository.

Document Description:
The current indexterm element cannot express the full range of indexing
semantics needed for production book indexes. This proposal addresses the
ability to express page ranges.
