OASIS Digital Signature Services eXtended (DSS-X) TC

During the f2f there was a request (I think by Konrad) for
a sample document in which a "visual verification report" is included as
additional page in a signed pdf-document. 
Another example of a (fairly straightforward) visual signature used in
German eGovernment may be found here:
