    This does not fulfill all the requirements you have listed, but there is a 
    project called EPAD that is a step in the right direction

    EPAD.
    
    EPAD-Emergency Provider Access Directory
    There is currently no accurate, comprehensive printed or electronic 
    directory of telephone numbers, Internet addresses, street addresses and 
    other relevant contact information for all federal, state and local 
    emergency response agencies in the U.S. This is a critical missing link in 
    the infrastructure of the emergency response community in America that 
    impairs the ability of these agencies to respond to individual or mass 
    emergencies.
    
    Quick and appropriate response to crisis situations affecting both 
    individuals and communities can be accelerated through a shared electronic 
    Emergency Provider Access Directory (EPAD). EPAD is a non-proprietary 
    directory, containing full agency contact information that will be made 
    available to all qualified public and private emergency response providers 
    in the nation. Rather than telling each agency what incident data they will 
    get and how, the EPAD will empower emergency response agencies by letting 
    them register:
    
      a.. How they want information delivered: Up-to-date computer, telephone 
    and fax numbers, addresses, jurisdictional boundary information, and an 
    information "sheet" for all emergency response and related agencies such as 
    hospitals, law enforcement and transportation agencies, emergency management 
    agencies, telephone carriers, telematics service providers, and the like;
      a.. What information they want delivered: The type of information, the 
    kinds of incidents, that they want to hear about;
      a.. For what areas they want notification for each kind of incident alert.
    
    EPAD will also list appropriate emergency contact information for private 
    entities which are able to notify the public at large.
