Doing similar counter-clockwise would not correspond to OOXML vert270. As you say, the tbrl case has multiple cases where the individual glyphs are or are not rotated. As far as I know, neither the libreoffice implementation of the proposal nor OOXML does glyph-level rotation in the btlr case (so that simplifies the situation).
