    I have replaced all occurencies of "unicode" and "UNICODE" with a 
    bibliographic reference to [UNICODE], that is defined as follows (see 
    page 697):
    
    [UNICODE]	The Unicode Consortium. The Unicode Standard, Version 4.0.0, 
    defined by:   The Unicode Standard, Version 4.0 (Boston, MA, 
    Addison-Wesley, 2003. ISBN 0-321-18578-1)
    
    Please note that this is a reference to the latest major version of 
    UNICODE (4.0). The XML 1.0 specification itself references UNICODE 3.2. 
    Please let me know if you consider this to be an issue.

    I have further added references to RFC2045 on some occurencies of 
    BASE64, and at the same time corrected the spelling of "base64" where it
